An employee was setting up a lane closure. A Truck Mounted Attenuator (TMA) was stationed in the roadway with the crash cushion in the downward position. The arrow board was mounted on the TMA in the upward position with the arrow activated instructing the traffic to move over. An employee was preparing to unload a truck mounted shipping container by activating the dock plate which is manually operated with a handle. While lowering the dock plate onto the back of the truck, the dock plate recoiled upon striking the right side of the truck causing the plate not to clear the truck as required to lay flat for unloading operations. The employee reached for a wrench that was stuck on the mixer and pinched the right index finger between the wrench and truck mounted pedestal.
