An employee was setting up a lane closure. A Truck Mounted Attenuator (TMA) was stationed in the roadway with the crash cushion in the downward position. The arrow board was mounted on the TMA in the upward position with the arrow activated instructing the traffic to move over. The pilot vehicle with a trailer attached was approximately 102 feet in front of the TMA. An employee was preparing to unload a truck mounted shipping container by activating the dock plate which is manually operated with a handle. While lowering the dock plate onto the back of the truck, the dock plate recoiled upon striking the right side of the truck causing the plate not to clear the truck as required to lay flat for unloading operations. The employee's left ring finger was pinched between the steel dock plate and left side steel edge of the dock plate.

An employee was checking bolts on a truck mounted rolling concrete mixer. The employee reached for a wrench that was stuck on the mixer and pinched the right index finger between the wrench and truck mounted pedestal. The initial injury was a severe laceration that later became a surgical amputation.
